We consider it a privilege to work together with you on the issues that are confronting you at this time. In order to make the first session as profitable as possible, we ask you to give careful attention to the following items.
If you have difficulty in completing any of the following steps, please call the church office and we will be glad to answer any questions you might have. The forms can also be obtained from the church office.
Carefully download and read "Facts about Biblical Counseling." Note any questions you might like to ask at the beginning of your first session.
Carefully download, read and sign the "Consent to Counsel" form.
Download and fill out the "Personal Data Inventory." Page 4 is very important and each question should be thought out carefully and then answered as concisely as possible.
Bring your completed forms into the church office and pick up a copy of "Christ and Your Problems." Read the booklet and highlight any comments in the booklet you find helpful.
